2.) Complete the Vendor Add Form For Non-Employee (office.com)
NOTE: If you traveled last year, you may have already completed step 1 & 2.
3.) Gather all the documents below for processing:
- Travel Worksheet for Non-DCF Employee
- All Receipts, including:

- Hotel
- Rental Vehicle
- Gas for Rental Vehicle
- Meeting Agenda.
4.) Submit all the information and receipts to HQW.OCW.TravelRequest@myflfamilies.com for processing.
